We consider applications for equipment from families and schools such as seats, standing frames, tricycles, powered mobility, computer equipment. An application is favourably considered if supported by a professional, e.g. a Physiotherapist or Doctor.
APPLICATION FOR FUNDING
If you wish to apply for a grant or donation towards the cost of a service, medical treatment or equipment please complete the contact form in the first instance.
To support your application we will usually request the following:
A covering letter describing the needs and the service or equipment required.
Supporting documentation from one or more professionals i.e. GP, Occupational Therapist etc.
Details of the amount you are requesting and the suPrivacy Noticepplier of the service or equipment.
Any further information you feel would be helpful in coPrivacy Noticensidering your request.
